An average General Surgeon in the United
states will face during his/her practice
a substantial number of proctology cases
which will probably average around 4 - 5
cases a month, and most of us - General
Surgeons - will perform them in a
satisfying traditional ways using what
we learned during our residency and
years of practice...This applies even to
busy Colorectal surgeons, may be with a
slightly higher number of cases.
For
some, yet unclear reason to me , and
although I am a General and Vascular
surgeon by training, my practice which
stayed fairly busy for most of the 16
years after finishing my training, took
for the last 5 years a different , more
specialized path when I developed a
sudden interest in performing better
less painful proctology surgery.
I always thought that
the way we used
to perform hemorrhoidectomy and other
proctology procedures is too primitive,
rough and inhumanely painful and does
not rise to the high tech advanced world
we live in today or the advances we
experienced recently in the field of
surgery.
So I started to improve on what is
known as the gold standards, I didn’t
invent anything, I simply started to use
the new equipments and the collective
knowledge I gathered through the years
from my Vascular and general surgery
background , in attempt to achieve a
simple goal : “treating proctology cases
especially Hemorrhoids effectively with
minimal or no pain”.
I found the
surgitron unit extremely
handy, gentle on the tissues, precise
and leaves much less pain
postoperatively than regular bovie or
cold knife.
I also found that the
Redfield IRC
unit when used properly and in a
slightly different setting than the
traditional recommendations is a very
effective, complications free tool.
Then came the
Ligasure which solved
my last problem with advanced stage 4
hemorrhoids, and made it possible for me
to perform all hemorrhoids cases even
the most advanced and complicated in my
office, under local anesthesia with some
sedation, in minutes...with the patient
going back home in half an hour and
going back to work the next day.
It was only natural that the
improvements will spread to nearby,
related topics, such as anal fissure and
perianal fistula and Pilonidal cyst
cases.
It was pure reputation and minimal
advertising that propelled my practice
to take off and get overwhelmingly busy
with proctology cases, patients told
other patients, and then referring
Doctors started to hear that I am
performing painless yet extremely
effective treatments of hemorrhoids and
related proctology cases in my office,
on an ambulatory basis with very good
results...so they started to
test me with few referrals in the
beginning...and when they sow the
results I quickly became their main
“proctology man”...It happened so
quickly that I had to renovate my office
and hire a CRNA and an extra nurse to
handle the extra load of patients.
For the last five years, I have been
doing an average of one hundred
proctology case a month, I devoted three
full days a week for proctology cases
alone...and this practice is still going
strong with no end in sight.
A year ago, I started to think that
this wealth of experience should be
shared with other fellow Physicians, it
is part of my duty and
responsibility...Then I thought that I
can do more than just a journal
publication or research, I can actually
teach - or show - other surgeons or
Family practioners how they can do it
....it will add a great dimension to any
physician’s practice, and it sure will
add to his/her income.
After all,
deep experiences make
things simpler, more effective and much
safer...so not every Physician has to go
through the same experiences to reach
what is right to do, we should learn
from each other’s experiences and freely
exchange these results...The new digital
world made this possible and relatively
easy....So
I decided to film my
procedures on a daily basis...and I took
permission from every patient after
explaining the final goal of filming the
procedure...and in less than a year
during which I had to buy large amounts
of storage media and work on collecting
the best and most representative of
cases , then edit them to keep the most
useful portions of every operation while
keeping it as “ live “ as possible and
adding my comment and then organizing
every thing into categories and
conditions and writing the steps and
details from A to Z , with one simple
goal in mind:
“That is to make it possible for
every physician who purchases this final
product to start from the very next day
to build a successful proctology
practice or to add this rich dimension
to his/her already busy or not so busy
practice”.
I took care to describe every detail,
every instrument and every technique I
use, I recommended what to buy in term
of equipment and how to care for it and
I filmed the operations with a
high
resolution Camera that shows thing so
clearly that you feel like you are their
with me during every step of the
procedure.
Among the 950 procedures I filmed I
chose about 200 to edit which I found to
be clearer and more effective in my
purpose of teaching what I do...No
attempt was made to hide mistakes or
mishaps...on the contrary…I was looking
for them and trying to show them...but
you are going to find soon after you
start watching the course that the
steps and techniques of every procedure
I cover are so simple, safe and
effective that you will notice a good
routine to every operation that will
make mistakes or mishaps extremely rare
and hard to find.
After all, this is not my first
attempt in publishing and teaching,
Since 1994 and until today, my book
“Safe Answers” for the American Board of
Surgery Certifying Exam has been the
most popular and widely used text for
the preparation of General Surgeons for
their Certifying oral exam and
recertifying written exams. I update my
book every year with new information and
new questions in the form of
presentations similar to what you would
face in your daily practice or the oral
Board exam and I wrote their safest
answers, based on the most updated and
solid references available to us.
It was probably the success of my
book “Safe Answers “that encouraged me
to venture into making this course...,
In my mind, I had to answer my self the
question:
Would I pay about a five hundred
dollars to learn in few days at the
comfort of my home the best and most
effective and painless techniques used
today in treating what represent more
than 10% of the cases I face every day
in my office i.e. proctology cases… And
from a surgeon who happens to do this on
a daily basis for the last 5000 cases
and is showing live his surgical
techniques with 10 to 20 examples for
every step..?
My Answer was Yes...based on a simple
fact...we pay for attending meetings and
medical conferences several thousands
dollars every year hoping to learn and
enrich our knowledge with something that
will add to our success and income and
serve our patients better...so why not
pay five hundreds to bring this success
to my doorstep, and have what I can
watch and listen to every day and when I
want and where I want ...and ultimately
be able to go the next day and apply
what I learned in my practice ?
The cost of all equipments and
instruments I use in my proctology
practice does not exceed One month
income of an average Physician, in my
judgment , this will be one of the best
investment that you could ever make to
serve your career, not only because you
will be able to build a very profitable
proctology practice on it, but because
many of these equipments like the IRC
and The Surgitron have multiple uses and
applications , and soon you will find -
like I did - that these equipments will
soon become so addictive that you would
want to use them in almost every case
you do.
